Cancellation

Sometimes life gets in the way and you need to cancel your gym membership. We’re sorry to see you go and hope that you’ll join us again in the future.

Cancellation of Services:

PIF: If you paid for your sessions in full, there is no cancellation fee.

General: After the 4 day period, client may cancel the remainder of this contract by paying: 1) One month of monthly dues or 2) 15% of remaining contract dues, whichever is less. Cancellation effective date will be the day client submits in writing (text, email, paper) that they wish to cancel. Client will receive an electronic cancellation form from PHP and client is required to submit form and pay cancellation fee within 3 days to complete cancellation. If client fails to submit form and cancellation fee the effective date will be the day form and fee are received.

Relocation: Should Client relocate his/her residence farther than twenty-five (25) miles outside PHP service area, and should PHP be unwilling to come to Client’s new residence because of its distance from his service area, Client may cancel this contract and shall be liable for only that portion of the charges allocable to the time before reasonable evidence of such location is presented to Progressive Health Pro Inc., plus a contract termination fee of 10% of the unused balance or $50, whichever is less.

Medical Leave: Should Client become unable to use or receive services under this contract due to death, disability, or under doctor’s recommendation, Client, or Client’s estate as the case may be, shall be liable only for that portion of the charges allocable to the time prior to death or the onset of disability. PHP requires reasonable evidence of death, e.g. death certificate, or disability, e.g. doctor’s note, to be presented at the time of cancellation.
